# Monthly active patients (MAPs)

A **monthly active patient**, or MAP, is a patient for whom Awell Orchestration performed an activity
that month. Awell bills for Awell **Orchestration**, not for Awell Studio.

So designing, validating and previewing care flows costs nothing in MAPs; running them for patients does.

## One Orchestration action makes a patient active for the month [#one-orchestration-action-makes-a-patient-active-for-the-month]

A patient is active if Awell Orchestration did **at least one** of these for them:

1. Started a care flow
2. Activated an action
3. Triggered a reminder

## A patient can be active without doing anything [#a-patient-can-be-active-without-doing-anything]

> **The patient does not need to have done anything themselves.** Sending a reminder marks that patient
> as active.

A long-running flow that sends monthly reminders to a dormant population keeps that population active,
and billable, whether or not anyone responds.

If a cohort has genuinely finished,
[stop their care flow](/docs/run-care-flows/stop-or-delete-a-care-flow) so those patients stop counting
as active.
