Edit a form's questions

Change, reorder, or delete questions and answer options in an existing form, after checking every care flow the form is used in.

This page covers editing the form itself: its questions and answer options.

Read this before changing anything. A form is shared, so changes to it appear in every step and every care flow that uses it, across the whole organization. Check where a form is used before editing it, especially one built by someone else.

Edit a question

  1. Double-click the step and select the form action to edit.
  2. Click Edit form to open the form builder.
  3. Select the question to change.

Reorder or delete questions

  • Reorder: drag and drop questions into place.
  • Delete a question: click the three dots on the selected question and choose Delete.
  • Delete one answer option: open a single- or multiple-select question, then click the trash icon next to the answer to remove.

Deleting a question deletes its data point

Anything downstream that reads that data point, such as conditional logic, a calculation, or a track trigger, then has nothing to read.

If a form is already live and collecting data, prefer adding a new question over deleting an old one.
